We are looking for a Bi-lingual HealthCoaches to join our team in Farmington!

Shift:

• Monday-Thursday: 11:30 AM to 8:00 PM and Saturday: 6:30 AM to 3:00 PM

Duties and Responsibilities

• Work with your assigned members to support their health journey utilizing the Tula program
• Outbound and inbound calls for scheduled calls and unscheduled support
• Manage real-time chat support with members
• Documenting member progress
• Coordinating appointments
• Adhering to HIPAA regulations
• Utilizing techniques in Motivational Interviewing
• Continuous diabetes education and other professional development
• Regular training sessions

• Effective communication and active listening for member support.
• Employing empathy, motivational interviewing, and behavior change models.
• Providing emotional support and respecting cultural diversity.
• Collaborating with healthcare teams and fostering a positive work culture.
• Adapting to policy, procedure, and technology changes with flexibility.
• Prioritizing care tasks and managing time efficiently.
• Upholding ethical standards, confidentiality, and patient autonomy.
• Handling stress and high-pressure situations with support and composure.
• Staying informed on diabetes care advancements and engaging in professional development.
• Complying with healthcare regulations and accurately documenting care.

Qualifications

• High School Graduate or GED.
• Bachelor’s degree preferred, or equivalent experience.
• 2+ years of experience in customer service.
• A passion for helping people.

Pay: $24.00 - $26.00 per hour
